Mostly you better not use your credit card in an ATM, as most credit card companies see this as a cash advance and charge you a lot extra for it.
Better take your normal bank card, just make sure your card can be used abroad and your bank expects use in Czech Republic. Also tell your credit card company you will be abroad.
When you use your credit card in a shop there will be a small surcharge, but the exchange rate is mostly a good one, so you will end up paying less than if you took the money in dollars to an exchange office, changed there and paid cash.
I always use a combination of ATM-bank card for cash to pay the smaller amounts and credit cards for the bigger amounts.
There are many ATMs all over Prague and you will find them in any city and even many of the villages, and I have noticed that not many, if any, of the bank ATMs charge a fee. It is likely your own bank and credit card company that will charge the fees.
