1) The lady at the consulate in Chicago advised me to go to Sears, 2 N. 2) Have a prepaid UPS envelope addressed to yourself with you so that they can mail you your passport when it is complete.

4) Leave the document you are required to fill in unsigned until you get to the consulate and sign it in front of them.
